R A S Home Health Care delivers nursing and home health aide services to seniors and people whose age, disabilities, or physical problems keep them from doing the normal, day-to-day living tasks they used to do.
Who qualifies for home care services?
To be eligible to receive RAS skilled home care services, patients must meet several criteria, including:
Have a homebound status (difficulty leaving or moving about without assistance).
Must be under the care of a physician.
Must require additional care following a recent hospital or nursing home discharge or as advised by his/her physician.
Any auto injury or injury at work.
To ensure the appropriate level of care, we will coordinate with you and your family to schedule an assessment interview. Our Nurse will make an in-home or in-hospital visit to share information about our services, whether it can be medical or non-medical, discuss the background and experience of our caregivers and establish a plan of care designed to meet your specific homecare needs. The Nurse will orient and supervise the assigned caregivers, as well as perform routine home visits, to ensure your care requirements are being met.
